Gather your information and documentation (optional)

• You may include any documents you feel support your claim, i.e. receipts, photos, and medical records.

 Fill out the Claim for Damages form

 Submit your claim in person or by mail to: City of Pomona, City Clerk's Office, 505 S. Garey Ave,   Pomona, CA 91766

 Submitted claims are forwarded to the Risk Management Department for processing.  Approval or   denial of your claim may take several weeks.
